The actual game is rhythm, not reflexes exactly. Chop steadily and the belt speeds up, which sounds like a problem until you realize faster also means more points per second if you can keep your timing clean. Slice money buys new knives, and later levels swap fruit for stranger material entirely — read a few reviews and people genuinely don't agree on whether the pizza levels or the produce levels are more satisfying.
